Application
FREE may approve your request in full or partially depending on available devices or funds. FREE recycles rehab equipment. Our inventory is dependent on community donations. To qualify, applicants must prove need for the device and limited resources.
The approval of your request is dependent on several factors. Applications remain active for a thirty day period. FREE is considered a provider of last resort and provides assistance only when all other resources are unavailable.
Please note that a physician's prescription must accompany all applications. If your request is for equipment valuing over $500.00, you are required to submit a "Letter of Medical Necessity" from your physician (a form is provided in this application).
